Nursing Care Plan is a process
comprises of 5 major elements that initially start with ASSESSMENT (data
collected from a nursing framework), nursing DIAGNOSIS (clinical judgment to
responses in actual or potential health problems of an individual, family or
community), realistic PLANNING (intermediate stage), nursing INTERVENTION
(focused on the etiologic or risk factors of the identified nursing diagnoses)
and EVALUATION of care.
